Oncology Screening Nurse Navigator--Lung Cancer Focus

Full Time - Day Shift

Hamburg Location, Lexington, KY

Job Description:

  • Assesses patient's educational, physio/psychosocial needs, barriers to care and support systems
  • Facilitates information gathering and analysis for patients to make informed decisions
  • Collaborates with multidisciplinary team members to facilitate timely access and continuity of care across various care settings
  • Develops and drives initiatives to eliminate barriers to care
  • Facilitates access to healthcare and community resources for patients as necessary
  • Individualizes the care of lung cancer patients throughout the continuum
  • Encourages patients to maintain records of their health history
  • Facilitates high quality, timely, and individualized patient care through coordination, collaboration, and advocacy
  • Maintains necessary information systems and processes to promote effective development, management, and reporting of the lung screening/cancer program
  • Serves as a liaison between the organization, patients, families, physicians, and the community for lung cancer care

Minimum Education, Experience, Training and Licensures Required:

  • Current KY or Compact State License as a Registered Nurse
  • Bachelor of Nursing
  • Certification in Oncology Nursing (OCN, AOCN, AOCNS or AOCNP) within one year of employment
  • At least 2 years of oncology experience
