The Public Health Nurse is an essential team member who provides public health nursing services to promote, improve, or maintain health and well-being for families, children, and adolescent populations in Dakota County through family health programming. This role does not provide direct care in a clinic setting.
Location & Schedule
This hybrid position offers a mix of telework, in-person work at our Western Service Center in Apple Valley, MN, and work with community throughout Dakota County. Work is performed Monday through Friday during regular business hours (8:00 a.m. to 4:30 p.m.). Occasional evening and weekend community outreach opportunities may be available.
What You'll Do
As Public Health Nurse, you will conduct nursing assessments of family needs and assist with connection to resources via in-person visits, virtually or phone. You will create and plan engaging evidence-informed health-related educational outreach activities and materials for children, adolescents, and families. You will provide outreach services and trainings to community partners, residents of Dakota County, and health professionals about preventative medical and oral health, and a variety of health-related topics. Outreach opportunities may include in-person events, safety camps, and monthly individual phone calls as a part of the Child and Teen Checkups program. This role does not provide direct care in a clinic setting.
This position will also work with our Intake program using critical thinking and attention to detail to assess and process referrals for family health programming using multiple computer software programs. You may also work with family health programs such as Child Passenger safety, Lead Case Management, Follow Along, or the oral health initiative to name a few! You will participate in emergency response activities as outlined in the department emergency response plan and other duties as assigned.
