Position Summary

The Direct Support Professional (DSP) Supported EM

mployment Team provides on-the-job support and training to adults with disabilities. Float to various external and internal job sites for team coverage. Provide written reports and information on each individual served.


Responsibilities

  • Communication - Speak clearly; listen to others; write effectively; establish relationships; act with integrity; establish trust.
  • Maintain effective working relationships with other Rise program staff that may share the same SET site-working environment.
  • Build and maintain effective working relationships with site contacts and maintain business partner's expectations for productivity, quality, etc.
  • Seamlessly fill in at multiple SET sites, some of which are single-staffed, and maintain good communication with site contacts.
  • Capable of working independently and adapting to any environment while maintaining a professional demeanor.
  • Ensure compliance with all applicable legal requirements and accreditation standards. Ensure that all department activities are conducted in a manner that is consistent with Rise's Code of Conduct, CARF standards, and HIPAA policies and procedures.
  • Observe and record work performance in accordance with established norms for designated vocational programs.
  • Maintain a high level of flexibility and is accepting of a fluid work schedule to meet program needs.
  • Develop and provide services based on individual goals identified by the IDT team.
  • Provide training and services in natural community environments.
  • Attend all DTH team meetings as scheduled. Share with the team updated information on individual needs, concerns, and employment site issues.
  • Develop and write training objectives based on the IDT's response to goal areas.
  • Develop methods designed for the persons served' learning abilities to carry out the training objectives, tracking the progress of the person served on the goal/objective form.
  • Write annual reports, quarterly reports, career plans, abuse prevention plans, case notations, progress reports, and incident reports, when needed, for one to three individuals.
  • Carry out self-care and support services (toileting, feeding, personal hygiene needs) as designated by the daily schedule.
  • Communicate with other program staff and residential staff on a daily or as-needed basis regarding program issues, caseload issues, staffing, and other related information.
  • Provide transportation services as designed by the driving schedule, drive and/or ride along on route.
  • Assist other staff with transferring of persons served from the vehicles.
  • Provide therapeutic programming as recommended by TO, PT, and speech therapists.
  • Assist in general cleaning and caretaking of the building (loading the dishwasher, doing laundry, picking up work materials at the end of the day, cleaning and maintaining the refrigerator, etc.).
  • Conduct oneself as a positive role model for persons served in terms of appearance, socialization, and teamwork.

Required Education, Experience, and other Qualifications

  • Technology skills: Proficient with Microsoft Office software (Outlook, Word, etc.).
  • Current, valid driver's license.
  • A driving record that demonstrates safe, legal driving habits.
  • High school diploma or GED Equivalent.
  • Compliance with Rule 11 and MVR background check required.

Preferred Education, Experience, and other Qualifications

  • Previous experience practicing person-centered practices with people with disabilities or other barriers to employment.
  • Production/manufacturing experience.


Working Conditions and Environment

  • Hours of Work: Hours of work are typically daytime, Monday through Friday.
  • Travel: Must be able to travel to various locations throughout the day, in the Twin Cities area.
  • Work Environment: This job operates in a variety of environments including Rise's office environment and production floor. The noise is moderately loud when on the production floor.
  • Physical Demands: The team member frequently is required to twist, stoop, bend, reach, pull, push, stand, sit and walk for up to eight hours per day. Able to lift and transfer people, with varying weights (50-75 pounds).

  • Hiring Bonus: $1,000 ($500 after 6-month work anniversary and $500 after 12-month work anniversary)
  • Wage: $20.40 - $21.42 per hour, depending on qualifications
