Overview

Pay range for this position is $29.62 - $43.07/hour. Actual pay is based on years of licensure.

POSITION SUMMARY: The RN- Population Health protects, promotes, and restores health and wellness, by practicing at the highest level of licensure; collaborates with providers and multidisciplinary team members to provide care coordination across the continuum; assesses and supports physical symptoms; provides psychological support to patients, friends, and families; mentors assigned team members; promotes best practice; maintains professional disposition at all times. The RN- Population Health oversees the management of patient care (specifically MeridianHealth patients), in collaboration with the Physician/Provider, around patient care needs/gaps focused on quality outcomes. Develops individualized plan of care with specific goals and targets including progress towards goals. The RN- Population Health is to coordinate preventative care and health screening activities. In addition, the RN works to highest level of licensure by providing quality competent care, following best practice standards, working to achieve care goals and quality outcomes through utilization of patient outreach to reduce care gaps, impact Nurse Sensitive Indicators, and improve patient outcomes. The RN will visit with the patient and independently administer medication and treatments per protocol/provder order and utilize nurse judgement and crticial thinking skills to provide quality care and encourage changes in patient behavior by promoting independence through goal setting with education and utilization of teach back methods.


Qualifications

REQUIRED QUALIFICATIONS:

  • Education: Bachelor's degree in nursing.
  • Experience: 6 months experience as RN in ambulatory healthcare setting.
  • Licensure/ Certification: RN in the state if IL
  • American Heart Association (BLS for Healthcare Providers)
  • Complete Preceptor Program within 6 months of hire
  • Successful Completion of OSF approved Nurse Clinic Education Courses
  • Ambulatory Care Nursing Certification required within 1-2 years of hire
  • Other Skills/ Knowledge: Excellent interpersonal and communication skills.
  • Solid computer skills, including proficiency with Microsoft software.
  • Strong analytical and problem solving skills, with the ability to be detail oriented.
  • Ability to regularly and timely travel between various sites.

PREFERRED QUALIFICATIONS:

  • Certification: Certification in area of specialty
