POSITION SUMMARY

The Director of Community Health & Wellness (Director) is responsible for the overall vision, planning, service delivery, coordination, and management of CLUES’ community health and wellness programs and initiatives in the Metropolitan and Greater Minnesota areas. The Director works with organization managers/leaders to reach and connect low-income and immigrant Latino families and other BIPOC populations to preventive healthcare, equitable food access and nutrition services, health education, health and social services navigation, and tools and resources that address social determinants of health. The Director ensures programs and services of the Community Health & Wellness Department further CLUES’ efforts to advance social and economic equity and wellbeing for Latinos in Minnesota. 

The Director provides leadership in setting strategic direction, in alignment with CLUES Strategic Framework, promoting integration and collaboration, and fostering innovation to move populations to achieve health and wellness. The Director oversees delivery of outcomes and implementation of programs that aim at meeting community needs. The Director develops proposals and manages state and federal grants and contracts. The Director develops relevant strategic partnerships with higher health and social services partners, public and private funders, and other relevant groups to facilitate and advance strategic organizational goals.

The Director of Community Health & Wellness works with Program Leaders and staff in a collegial and supportive manner to ensure that all wrap-around services meet client needs and are linked appropriately to programs and services across the Agency in alignment with strategic goals and objectives. The Director is a member of CLUES’ leadership team.

Schedule: Full-Time. Generally, Monday – Friday Office hours. Some evening or weekend hours may be required. 
Estimated Salary: $80,000 to $100,000 (depending on experience & qualifications, open to negotiation).

ESSENTIAL DUTIES AND RESPONSIBILITIES

Program Development

  • Develop innovative approaches to address social determinants of health that lead to improving population health through programmatic approaches in areas such as health education, tobacco cessation, nutrition, equitable food access, disease prevention, health screening (cancer, HIV, etc.), sexual health education, and access to health care services (e.g., health insurance enrollment), in a variety of settings including onsite, home visits, community and partner sites, health fairs, and community events.
  • Create a supportive, participant-centered environment to effectively respond to their individual and family needs. Ensure the delivery of services is linguistically and culturally appropriate.
  • Lead staff to achieve program outcomes and deliverables. 
  • Represent the organization with local funders, state, and federal agency networks and partners. Ensure needs and opportunities for community health & wellness for Latinos are prioritized.  
  • Develop, analyze, monitor, and report department goals and outcomes, ensuring their alignment with overall mission, values, and strategic goals of the organization. 

Fundraising and External Relations 

  • Cultivate donor relations and build new funder prospects. Maintain an understanding of current industry and regional economic ideas, trends, and practices pertaining to this Division.
  • Build and strengthen strategic partnerships with local, regional, and national foundations, corporations, and government entities. Advice the Development Team on fundraising and community impact trends. 
  • Research and advice on external platforms that can raise the profile and visibility of the organization.
  • Represent and promote the work mission, vision, and values of CLUES as it pertains to Community Health & Wellness.
  • Work with CLUES President to ensure policy, advocacy, and legislation are evaluated and leveraged for maximum community and organizational impact.

Leadership

  • Develop and sustain an effective and productive working partnership with the VP of Programs in managing programs and achieving strategic goals and outcomes. 
  • Provide program-related recommendations to Leadership.
  • Promote collaboration, staff initiatives, and create growth opportunities for understanding program and special project management.
  • Serve and actively participate as a member of the Leadership Team.
  • Establish and monitor standards of excellence and consistency for program management practices throughout the organization. 

Team Building and Development 

  • Hire, develop, coach, and retain high-performance team members, empowering them to elevate their level of performance and accountability for achieving expected results, while ensuring their mutual support, learning, and sharing. 
  • Provide support to ensure staff’s productivity and performance, while assuring a positive customer service-oriented work environment. 
  • Develop and implement strategies that maximize synergies among programs for targeted constituencies.
  • Develop the necessary systems, processes, and tools to better support the facilitation, collection, and sharing of knowledge that is generated by individual programs. Develop dissemination system to share organizational learning.

Financial Management, Compliance, and Quality Assurance

  • Secure grants and contracts to advance programmatic work ensuring long-term program sustainability, with support of division leaders and Development department as appropriate. 
  • Maintain compliance with funding sources requirements and ensure accurate and timely report submission. 
  • Maintain appropriate fiscal and expenditure controls. Develop annual budget in collaboration with division leaders and fiscal department staff. Monitor performance against the annual departmental budget and goals. 
  • Maintain processes to collect relevant data to track and report upon outcomes in a timely manner according to grant/contract requirements and department goals. 
  • Implement process improvement as needed. 
  • Develop and monitor key indicators and business model drivers against which to monitor progress and service effectiveness.
  • Develop, monitor, and evaluate work plans, processes, systems, and procedures to achieve program goals while meeting quality and service expectations.
  • Ensure staff maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations, and ethical standards.

General Duties

  • Work with Marketing to create plans for communications and outreach according to local/individual needs.
  • Partner with other leaders to effectively deliver integrated client services. Ensures that Division’s staff collaborate proactively with other programs in CLUES to provide a comprehensive array of services to clients.
  • Provide leadership for programs and services in multiple locations as directed (including the metro area and Greater MN). 
  • Perform all other duties as assigned by supervisor.

SUPERVISORY RELATIONSHIPS

  • This position is responsible for supervising employees, the program and/or department leaders and selecting staff within the Community Health & Wellness Department. 


QUALIFICATION REQUIREMENTS

Education, Experience & Key Abilities*

  • Bachelor’s degree and 3-5 years of experience in public health, social services, or related field required. Master’s degree in related field preferred. 
  • Strong financial program management experience required.
  • Five (5) years or more of management and professional staff supervision experience required.
  • Bilingual in English and Spanish and cultural sensitivity and awareness required.
  • Must have a working knowledge and experience in the following areas: 
    • Principles and practices of organization and project, planning, development, and management. 
    • Leadership principles for strategic short and long-term planning. 
    • Leadership principles for communication and engagement with leaders in the private and public sectors in related businesses and industries. 
    • Effective program and grant design, planning, development, and budgeting in response to complex RFPs, proposal requirements, and specifications.
    • Regional and state community health and wellness trends, pathways for addressing social determinants of health, prevention, and health education.
  • Must have excellent interpersonal communication skills, including sensitivity and awareness regarding the Latino community’s culture and traditions. Must be able to connect cross culturally.

Language Skills 

  • Excellent oral and written communication skills in English and Spanish required. 
  • Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals. 
  • Ability to effectively present ideas and information in an organized and effective manner, including writing reports. 
  • Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management, and external parties. 

Transportation 

  • Must have a reliable motor vehicle for independent transportation, valid driver’s license, proof of automotive insurance, and maintain a good driving record. Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.

Computer Skills 

  • Proficient working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required. 
  • Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties. 

    

WORKING CONDITIONS

Background check

  • Must pass a criminal background check and driving record check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these, and additional background checks.
