POSITION SUMMARY 

CLUES’ Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos, will be providing immediate access to supportive, high-quality, culturally-responsive and integrated Behavioral Health services and pathways to wellness for the Latinx Community, regardless of insurance status or ability to pay. The CCBHC model provides CLUES with a model for sustainable Behavioral Health service provision, which translates to increased access to consistent care for wellness in the Latinx community. 

CLUES’ LPN role is essential for supporting our clients in Medication Assisted Treatment (MAT) treatment, which is treatment of addiction with medication. The LPN would provide in-person, on-site supportive services to clients who need medication for the treatment of addiction to opioids, alcohol, or other drugs. The LPN would provide initial and ongoing assessment of vitals and reactions to medications, in collaboration with our MAT provider and our psychiatrist, who is the Medical Director. The LPN would also similarly support the work of CLUES’ psychiatrist in their general psychiatry practice. The LPN would assist clients with medication refills, in collaboration with the MAT provider and the psychiatrist. This LPN would also be in charge of collecting required primary care screening data, which is a requirement of the CCBHC.

Position Type: Regular 
Schedule: Full-Time. Generally, Monday – Friday Office hours. Occasional/Some evening or weekend hours may be required.  
Estimated Pay: $26.00 to $28.00 (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Conduct primary care screening for Behavioral Health clients as a requirement of the CCBHC. 
  • Support clients receiving services from CLUES’ MAT provider. 
  • Support clients receiving services from a psychiatrist and/or future psychiatric providers. 
  • Take and record vital signs, such as blood pressure.  
  • Administer medications as ordered and report adverse reactions to medications or treatments.  
  • Take necessary labs and conduct routine physical health screenings in conjunction with MAT and psychiatric services.  
  • Monitor clients and chart medical records accurately and in a timely manner.  
  • Report the condition of clients back to MAT and psychiatry provider(s).  
  • Communicate openly and effectively with clients and families, while demonstrating compassion and friendly behavior.  
  • Ensure that supplies, medications, and equipment are restocked and maintained and in ready-to-use condition.  
  • Provide support to clients to process prescription refills.  
  • Participate in Continuous Quality Improvement activities and support by collecting data in the Electronic Health Record and other data sources to help the Behavioral Health department improve services to the community. 
  • Cultivate working relationships with CLUES staff within and across departments, to support one another’s work, facilitate client referrals, and improve client flow throughout the agency. 
  • Respond to all internal & external data collection & reporting requests. 
  • Contribute to the collaboration of the Behavioral Health department with other programs within CLUES to achieve service integration.  
  • Support positively the mission, operating values, and services of CLUES within the community.   
  • Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards. 
  • Carry out all other duties as assigned by supervisor.

QUALIFICATION REQUIREMENTS 


Education, Experience & Key Abilities

  • Must be a graduate of an accredited LPN nursing program.  
  • Must have a valid LPN license in the State of Minnesota. 
  • Must have excellent communication and interpersonal skills, including the ability to work with clients and family in a respectful, warm way.  
  • Ability to demonstrate good judgment and decision-making skills.  
  • Ability to stand or walk for extended periods of time and lift medical equipment.  
  • Ability to demonstrate resilience in emotionally difficult situations.  
  • Must have a deep understanding of Latinx cultures, values, and traditions, and demonstrate cultural responsiveness in every aspect of the role, including communication with colleagues, staff, clients and community members; creating forms and documents; training staff; and in clinical supervision and consultation.  
  • Strong attention to detail and ability to ensure that outgoing information is thorough and accurate. 
  • Must collaborate effectively with co-workers and key stakeholders as a “team player”. 

Language Skills  

  • Effective communication in Spanish and English, both verbally and in writing, required. 
  • Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals.   
  • Ability to effectively present ideas and information in an organized and effective manner, including writing reports.  
  • Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties.   

Mathematical Skills   

  • Ability to add, subtract, multiply, and divide in common units of measure at a high school graduate level minimum required.  

Reasoning Ability   

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 

Computer Skills  

  • Proficient working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required.   
  • Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties. 
 

WORKING CONDITIONS 

Background check 

  • Must pass a criminal background check, DHS background study, and MS-604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.
