Clinical Coordinator - Oxbow Lake
Location:Brooklyn Park, MN
Exempt/Non-Exempt:Exempt

Department:Nursing
Description:Saint Therese at Oxbow Lake is currently seeking a full time Clinical Coordinator for our Care Suites. The Saint Therese at Oxbow Lake Suites includes Memory Care, Transitional Care, and Long Term Care Suites. Some weekend and holiday exposure is required. Flexibility is preferred.
Duties:Implements a plan of care of each resident assigned. Completion of RAI process for assigned residents including assessments, implementation of care plan and care guide, and completion of MDS. Assures appropriate Medicare documentation is completed. Monitors documentation to support Case Mix. Assures that the integrity of the nursing practice of the nurses under his/her supervision is in compliance with the Minnesota Board of Nursing, Nurse Practice Act and standards of care. Maintains open lines of communication with residents, staff, families, volunteers and visitors. Administers human resources related policies, procedures and activities on the unit. Provides input to fiscal management of the unit to ensure proper utilization of human and financial resources. Maintains a pattern of professional development and improvement.


Understands and utilizes the nursing process to problem solve, make decisions, communicate, direct and evaluate resident care. Demonstrates professional and competent nursing practice. Promotes safety and follows all safety procedures and protocols. Participates in educational programs per State and Facility requirements. Performs all duties consistent with Quality Standards and Infection Control practices. Performs all job functions within State and Federal Regulations. Other duties as assigned
Qualifications:Education:
Successful completion of an approved nursing program for Registered Nurses. Certification as Geriatric Nurse preffered.

Experience:
At least 5 years nursing experience in LTC preferred. RNC beneficial.
Special Knowledge, Skills and Abilities:
Certified in CPR. Current Minnesota Registered Nurse licensure

Special Knowledge, Skills and Abilities:
Working knowledge of Federal and State rules and regulations related to long term care.
Strong understanding of and interest in palliative/hospice care
Professional behavior and attitudes
Demonstrated ability to make appropriate professional judgments
Excellent written, verbal and interpersonal communication skills
Working knowledge of personal computers and related software
Ability to follow direction of supervisor
