Description

Job Purpose: The Clinical Support RN supports the healthcare provider by administering phone and clinic based nursing care within the scope of nursing practice. The Clinical Support RN assists providers in management of medication refills and care for MNGI's sub-specialty patient population including inflammatory bowel disease, hepatology and other GI related conditions.

Key Responsibilities:

* Assess patients with symptoms resulting from their chronic condition. Suggest nursing interventions for symptom management based on nursing protocols and communicate changes in patient's status to the monitoring provider.

* Identify barriers to patient's compliance with recommended treatment plan and coordinate with members of the healthcare team to overcome barriers

  • Complete phone visits for appropriate patient populations as per protocol
  • Perform in-person nursing clinic visits to review appropriate use of new medications as well as perform health maintenance assessments.

* Provide patient education.

  • In collaboration with providers, and within RN scope of practice, assist in the development of protocols and/or written guidelines for Clinical Nurse activities
  • Follow physician-signed protocols for medication refills and symptom assessment.
  • Send prescription refills electronically using the electronic health record
  • Review and maintain accurate and detailed records in the electronic health record

* Serve as a resource for patients, ancillary staff and providers and function as a part of a team to achieve patient outcomes.

  • Attend and participate in sub-specialty focus group meetings as needed
  • Other duties may be assigned to meet business needs

Education and/or Experience: Associate's or Bachelor's degree in nursing is required. One to two years of clinical experience is required; One plus years of GI experience is preferred. Experience using an Electronic Health Record is required.

Certifications/Licenses: Current Minnesota Nursing License is required.

The employee works in an office environment and clinic environment and is infrequently exposed to hazardous chemicals or conditions. The employee may be required to travel to all MNGI sites. The employee is exposed to a computer monitor daily.
