Job Description

POSITION SUMMARY

The Manager of Community Health & Wellness (Manager) is responsible for integrating and advancing Health, Wellness, and Food Access programs and grants in the Metropolitan and Greater Minnesota areas. The Manager will work with organization managers/leaders to connect low-income and immigrant families to preventative healthcare, equitable food access, and tools and resources that meet their basic needs. They are responsible for assessing and creating new health and wellness programs that advance equity and community engagement efforts in alignment with CLUES’ Strategic Plan.

The Manager of Community Health & Wellness’ primary role is to foster effectiveness, integration, and alignment of services with community needs; develop and cultivate donors/external relations, and create strategies that support outcome measures, effective community engagement, and development of work plans. They will provide supervision to department employees and ensure that all internal functions and services are linked appropriately and are in alignment with strategic goals and objectives in the strategic plan. The Manager works with program leaders and supervises Greater Minnesota Navigators role in related family support services.

Schedule: Full-Time. Generally, Monday – Friday Office hours. Some evening or weekend hours may be required. 
Estimated Salary: $60,000 - $65,000 (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES

    

    Department Leadership & Management

  • Provide oversight of health and community support services offered in Spanish & English in both the metro area and Greater MN. 
  • Oversee staff delivery of community health services to clients, including one-on-one client services as well as classes and workshops as appropriate. Work with staff to provide services in the areas such as general health education, tobacco cessation, nutrition, equitable food access, disease prevention, health screening (cancer, HIV, etc.), sexual health education, and access to health care services (e.g., insurance enrollments such as MNsure), in a variety of settings such as the office, home visits, community and partner sites, health fairs and other community events. 
  • Oversee, support, and further Community Health programs such as La Canasta Familiar ¬– pop-up food shelf and on-site pantry, Jardín de Armonía En Acción – Community Garden, COVID-19 Vaccination Clinics, MNSure & SNAP, etc. 
  • Manage short-term and long-term planning, implementation, evaluation, and reporting of mission-related, outcome-based community health and wellness programs.  
  • Provide leadership and direction to the staff in the department to promote an optimal level of service by consistent high-quality oversight.
  • Create a supportive, client-centered environment to effectively respond to the needs of clients and their families. Ensure the delivery of services is linguistically and culturally appropriate.

Program Development, Financial Management & Compliance

  • Effectively manage department finances, including program budget and resource allocations relative to financial targets and department goals, with oversight of VP of Programs. 
  • Work closely with VP of Programs and Development Department to maintain existing funding and apply for and secure new funding opportunities. 
  • Maintain processes to collect relevant data to track and report upon outcomes in a timely manner according to grant/contract requirements and department goals. 
  • Monitor staff productivity, performance measures, and program outcomes to ensure all programs in Community Health will achieve contract, department, and financial goals. Provide progress reports and corrective action plans when needed to meet or exceed financial and performance targets.
  • Understand and adhere to all laws, regulations, rules and policies applicable to services offered by department. Ensure compliance with funder/contract requirements and comply with auditing requirements. 
  • Maintain necessary certifications for department to provide services (e.g., MNsure Health Navigator). Monitor and facilitate staff receipt of certifications, as per agency certification and contract requirements.  
  • Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards while ensuring staff within the Community Health Department do the same. 

Personnel Management & Development

  • Collaborate with supervisor and Human Resources department to plan, implement, and manage staffing to maximize effectiveness. Recommend staffing structure and personnel decisions including hiring, promotion, disciplinary action and termination to supervisor and HR. Ensure staff have adequate training and necessary certifications or licensure when required. 
  • Effectively supervise department staff, including: hiring, providing guidance and direction, training, evaluating performance, initiating corrective action or disciplinary action and other personnel actions. 
  • Provide direction, training, and coaching to staff, including new staff on-the-job training and department orientation, etc. Mentor and encourage staff professional development. Provide constructive feedback as well as positive recognition. 
  • Designate individual productivity goals for staff based on department goals and hold staff accountable. Provide clear and timely communication to staff regarding expectations and delegated tasks. Maintain regular and timely documentation of staff performance. Meet regularly with department staff to oversee progress toward individual and department goals. 
  • Nurture a healthy, dynamic and team-based working environment. Develop a well-functioning team, fostering staff empowerment.

Community Outreach & Communications

  • Work with Marketing to create plans for communications and outreach according to local/individual needs.
  • Develops and oversees department \outreach efforts. 
  • Represent CLUES in fostering community health and wellness relationships in Metropolitan and Greater MN communities. Maintain relationships with community partners and identify new opportunities for partnership. 

General Duties

  • Partner with other leaders to effectively deliver integrated client services. Ensure that department staff collaborate proactively with other programs in CLUES to achieve service integration and provide comprehensive array of services to clients.
  • Promote the mission, operating values, and agency services of CLUES within the community.  
  • Manage services provided in multiple locations as directed (including metro area and Greater MN). 
  • Carry out all other duties as assigned by supervisor.

SUPERVISORY RELATIONSHIPS

  • This position is responsible for supervising employees, interns, and volunteers within the Community Health Department.  

QUALIFICATION REQUIREMENTS

Education, Experience & Key Abilities*

  • A bachelor’s degree and at least three years of experience in public health or related field required. Master’s degree in related field preferred. 
  • One to two years of experience in non-profit program management or other related management required.  Program development experience preferred. 
  • Leadership and supervision experience required. Must be able to foster a positive, team-based environment among staff. Must possess demonstrated ability to solve practical problems, manage conflict and change, foster teamwork, successfully manage interpersonal relationships, and provide leadership. 
  • Bilingual in Spanish and English with cultural sensitivity and awareness required. 
  • Knowledge of and demonstrated experience providing fact-based community health education and/or services required. 
  • Certification as a Health Navigator for MNsure preferred. If not already certified, may be required to successfully complete a certification course as required by the MNSure contract.
  • Food Safety Certified preferred. If not already certified, may be required to successfully complete a certification and approved registration with the Minnesota Department of Health.
  • Comfortable discussing health topics in a sensitive and appropriate manner. 
  • Skill in organizing time independently to complete multiple projects, deadlines, and program priorities required.
  • Must be organized, ability to work under pressure and stress, have attention to detail and the ability to multi-task in a fast-paced environment.
  • Ability to work both independently and as part of a team required. 
*Equivalent combination of education and experience may be accepted.


Language Skills 

  • Effective communication in English and Spanish, both verbally and in writing, required.
  • Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals.  
  • Ability to effectively present ideas and information in an organized and effective manner, including writing reports. 
  • Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties.  

Transportation 

  • Must have access to use a reliable motor vehicle for independent transportation, valid driver’s license, proof of automotive insurance, and maintain a good driving record. Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.

Computer Skills 

  • Proficient working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required.  
  • Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties.

Background check

  • Must pass a criminal background check and driving record check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.
