What You Will Do Like their hospital counterparts, the ACRN will be responsible for starting the patient's care which includes performing a comprehensive nursing assessment, medication reconciliation, safety assessments, and implementing standardized protocols. The difference is that this care is provided in the comfort of the patient's home. After the start of care visit, patients receive ACRN visits at least 2 times per day. During these visits, the ACRN will assess the patient's condition, administer prescribed medications (typically intravenously), obtain blood samples, provide therapeutic interventions, and educate the patient and family members on how to manage their illness; all with a goal of restoring the health and stability of the patient. During the morning visit, the ACRN will participate in telehealth rounds with the provider and patient. A Recovery Care Coordinator will work remotely with the ACRN to assure needed supplies are at the patient's house, arrange transportation and follow-up appointments, and follow-up with the provider as lab and test results become available. This is a great opportunity for an experienced RN that enjoys connecting with patients while providing highly skilled care. Because our ACRNs provide hospital-level care in the home, 3-5 years inpatient experience (preferably in med/surg) is required, and experience providing nursing care in the home, community, or clinic is highly desirable. An ASN or BSN from an accredited school of nursing and current license RN license in State are required.
Perform patient assessment and provide care
Educate patient on their condition, prescribed medications, nursing interventions, and care plan
Participate in the coordination of services with other disciplines and community resources as needed
Identify and act on changes in patient's condition
Evaluate and revise care plans/assessments based on changes in patients and/or their environments
Document all assessments and interventions
Collect specimens as required (including phlebotomy)
Start and manage peripheral IVs, midlines, and PICCs
Administer medications including IV medications as ordered
Insert foley catheters as ordered
Utilize and instruct patient/family on the use of the telehealth tablet used for biometric data capture, patient symptom reporting, and telehealth visits
Participate in regular case conferences and in-service or special case orientations as needed and participate in on-call rotation
Other duties as assigned.
Qualifications Required
ASN required
Currently licensed to practice in applicable State
BLS required
2 years of RN experience required, 3-5 years of RN inpatient experience (preferably in med/surg) is preferred
Preferred
BSN preferred
Experience providing RN services in the home, community, or clinic is preferred
