Regardless of role, we recognize that every member of our team is vital! The Social Worker Care Coordinator is a fully integrated professional and core member of the primary care team. This role identifies home healthcare patients and provides care coordination including: social service needs and serving as liaison between the patient, clinic, physician, and various community agencies. The person in this position assists the patient in understanding home healthcare delivery system and acts to resolve obstacles that prevent them from receiving adequate health care.

Hours: Part Time, 20 hours a week, Clinic Hours between 8am-5pm
