Salary range for this position is $124,592-$146,577; Actual pay will be determined by experience, skills and internal equity.

OSF HealthCare is excited to announce this new type of position. OSF is starting a PACE program (Program of All-Inclusive Care for the Elderly) - which is a program that supports high risk patients within Medicaid/Medicare population. The program will start small and we anticipate slower growth over the first 6 months due to the complexity of the program. We have set up this position to practice in a blended environment while this growth occurs. The individual will have the opportunity to practice in Medi-Park's community pharmacy practice, the e-Pharmacy oncall advanced care virtual clinic and the PACE clinic itself. % of time in all environments will flex as we grow. There is a high direct patient contact component to this position. We anticipate there also to be a 10% administrative component as we evaluate cost effective use of medications in the PACE population - because we are both the provider and the payor. While there is an "ePharmacy" component, the position is an in-person located in Peoria because of the percentage of work that will be in-person patient contact. Flexibility will be key to this new practice model - especially in its growth phase.

POSITION SUMMARY: Reviews prescriptions, obtains clarifications, analyzes drug therapy, and regularly consults with medical staff, nursing personnel and others in order to coordinate delivery of high-quality pharmaceutical services to OSF HealthCare patients and/or participants. Responsible for compounding and dispensing medications according to established procedures and professional pharmacy practice. Coordinates medication therapy management and medication adherence programs to optimize medication outcomes in specified patient and/or participant populations. Collaborates with patient and/or participants care teams and providers through interdisciplinary rounding and consultative referrals. Demonstrates the knowledge and skills necessary to provide patient and/or participants care that is appropriate to the ages of those served.

PRIMARY JOB RESPONSIBILITIES AND ACCOUNTABILITY:
Manages and improves the medication-use process by reviewing prescription orders and ensuring orders are safe, appropriate, cost-effective, and processed timely according to established policies, procedures, and protocols. Dispenses all medications efficiently and accurately.

Provides evidence-based, patient/participant-centered medication therapy management according to approved protocols of Pharmacy Services (pharmacokinetics, warfarin dosing, IV to PO, renal dosing, etc.) and recommends/monitors appropriate lab studies needed to determine whether therapeutic objectives are being obtained.

Prepares compounded medications according to standard formulas, ensuring compatibility and stability of prescribed additives in compounded preparations.

Oversees, assigns, and reviews the work of Pharmacy Technicians responsible for performing a variety of tasks (i.e., prescription filling, data entry, etc.).

Participates in multi-disciplinary patient/participant care rounding and assists other health care providers in identifying and resolving patient/participant care issues associated with drug therapy.

Documents all clinical activities and interventions accurately and completely. Completes claims submissions to meet payor requirements for billing of prescriptions and MTM services rendered.

Collaborates with and acts as a resource for providers, nurses, case management and other members of the patient/participant care team, including patient/participant counseling when required.

Exercises leadership and practice management skills by assisting in training and orienting new pharmacy personnel and educating physicians, residents, nurses, and students regarding pharmacokinetics, drug information, drug formulations, etc. Assists with academic functions such as serving as a preceptor for students, healthcare professional interns, and residents including PGY-1 and PGY-2 pharmacy residents.

Assists in adherence to department safety standards and reporting processes. Recommends solutions and improvements for safety problems in conjunction with leadership.

Demonstrates good organizational skills in coordinating services with Mission Partners and other departments/services. Responds to stressful situations in a calm and rational manner and cooperates with all to facilitate integration of services.



Qualifications

REQUIRED QUALIFICATIONS: Education: Doctorate in Pharmacy (PharmD)

Experience: 2 years of experience in hospital and/or retail pharmacy OR the equivalent experience gained through completion of a Pharmacy Practice PGY1 residency.

Licensure/ Certification: License (or eligibility for licensure) to practice pharmacy in the state of Illinois and/or Michigan, as applicable to the location of work.

Other Skills/ Knowledge: Interpersonal skills necessary in order to communicate effectively with physicians, patients/participants, and various other OSF HealthCare personnel when explaining medication use and gathering and exchanging other medication information. Analytical skills necessary in order to determine drug appropriateness and evaluate drug interactions. Demonstrated skills in independent problem solving, decision making abilities, and prioritization/delegation. Demonstrated knowledge of the principles of growth and development as well as the physical, emotional, and psychosocial needs of neonates, pediatrics, adults, and geriatrics. Excellent computer skills and the ability to process detailed information in an accurate manner. Competency in the use of an electronic medical record system. Compliance with state and federal pharmacy rules and regulations.
