South Metro Human Services has a full-time position that serves as a primary contact for the UCARE member and for UCARE management. Some job duties; develop and implement a comprehensive plan of care (POC) in conjunction with the member and family members or representatives according to required time-lines; conduct a comprehensive face-to-face assessment using the SNBC UCare Connect Assessment Tool and other clinically appropriate supplemental assessment tools. Candidate must have a minimum of a Bachelor's degree in Psychology, Social Work (with LSW) or behavioral science related to the field from an accredited college or university, plus 2,000 hours (1 year full-time) work experience with SPMI/CD adults. Hours are Monday-Friday day hours.
This position is eligible for full benefits including paid medical, dental and life insurance; 4 weeks Paid time off; 10 Paid Holidays; $80 cell phone and $80 parking benefit each month; mileage and tuition reimbursements. Salary is $36,500.
