Job Description: Children's Mental Health Case Management (CMHCM) provides in-home support and guidance for families with children up to age 18 (or 21 if in high school) diagnosed with a severe emotional disturbance who live in Hennepin County. Our Children's Mental Health Case Managers provide advocacy and support while also developing a care plan individual to the client. This care plan could include therapy, special education services or changes in a child's school routine, mental health skills training, social skills building and family support. Position Summary: St. David's Center is currently seeking a full-time Children's Mental Health Case Manager to join our Children's Mental Health Targeted Case Management Team. This position offers a unique opportunity to provide support and case management services to children and their families who are experiencing mental health challenges. The Children's Mental Health Case Manager is responsible for a variety of tasks, including but not limited to:
Schedule: M-F, 40 hours per week; requires some evening flexibility. Pay: Range depends on qualifications and experience. Qualifications: Education: Qualified applicants will have a Bachelor's Degree in the Behavioral Sciences field from an accredited college or university. Experience: Qualified applicants will possess a minimum of 2,000 hours of supervised experience in the delivery of mental health services to children and experience with families from diverse communities.
