Job Description:

Children's Mental Health Case Management (CMHCM) provides in-home support and guidance for families with children up to age 18 (or 21 if in high school) diagnosed with a severe emotional disturbance who live in Hennepin County. Our Children's Mental Health Case Managers provide advocacy and support while also developing a care plan individual to the client. This care plan could include therapy, special education services or changes in a child's school routine, mental health skills training, social skills building and family support.

Position Summary:
St. David's Center is currently seeking a full-time Children's Mental Health Case Manager to join our Children's Mental Health Targeted Case Management Team. This position offers a unique opportunity to provide support and case management services to children and their families who are experiencing mental health challenges.
The Children's Mental Health Case Manager is responsible for a variety of tasks, including but not limited to:
  • Assisting the child in gaining access to needed medical, social, educational, and other services to address their Mental Health needs
  • Coordinating services designed to enable a child to improve or maintain functioning in order to remain with his/her family in the community
  • Developing supportive relationships with the child, the child's family, and the team serving the child, while consistently maintaining professional boundaries and reflective practice
  • Collaborates with the family to plan services while focusing on the strengths of the child and of the family, viewing the child's needs holistically, and recommending services that will help the child reach or maintain his/her optimal level of functioning in a community-based setting.
Schedule: M-F, 40 hours per week; requires some evening flexibility.

Pay: Range depends on qualifications and experience.
Qualifications:
Education: Qualified applicants will have a Bachelor's Degree in the Behavioral Sciences field from an accredited college or university.

Experience: Qualified applicants will possess a minimum of 2,000 hours of supervised experience in the delivery of mental health services to children and experience with families from diverse communities. Prior mental health targeted case management experience is preferred.
Knowledge & Skills:
  • Strong skills in assessing and identifying the needs of children with mental health diagnoses
  • Basic knowledge of DSM IV-TR and DSM 5 diagnosis
  • Strong knowledge of community resources
  • Ability to work independently and with flexible work hours
  • Excellent written and oral communication skills
  • Strong organizational and time management skills, and attention to detail
  • Ability to work as an interdisciplinary team member
  • Experience with Word and Excel required
